Repeat Prescription Form

Please CLICK HERE to complete a repeat prescription form for your dog or cat. Once submitted, your request will be sent directly to your pet’s care team for review.

We will only contact you if we need further information. Otherwise, your prescription will be processed within 48 hours (Monday – Friday). If you plan to collect your prescription in person, please call ahead to confirm it is ready before making the journey.

Please note:

• If you require this medication urgently, you should call us directly on 01628 308330 – and select option 2 – rather than using this form.
• If your pet has not been seen by us in the last six months, a follow-up appointment will be required before we can prescribe further medication. Please call 01628 308330 – and select option 2 – to book an appointment.
